AAA version may be used with any contract. Provides for a fast,
easy and cost effective way to resolve disputes by an AAA
construction specialist rather than lengthy court
litigation.
Records a description of
material, man hours, labor, equipment and subcontractors.
Includes separate Cost Breakdown form to help organize and
cost-out all the necessary items in order to determine your time,
material, costs and profit. Both forms will automatically
calculate.
Lists employees on the
project, rate of pay, hours worked, work accomplished,
description of materials/supplies received and quantity used,
records of equipment, subcontractors, changes and
inspections/injuries/delays.
Three different formats to
choose from, establishing Mechanic's Lien rights for those

This form is to be used by
a party who applies for a progress payment when the progress
